In 2015, the airline was involved in a major incident when one of its Boeing 737-800s crashed in the Pyrenees Mountains. The accident resulted in the death of all 144 passengers and crew on board. The cause of the crash was later determined to be pilot error due to fatigue.

In 2017, AlbaStar was the subject of a federal investigation into its safety practices. The investigation revealed that the airline had failed to comply with safety regulations and had not performed regular maintenance on its aircraft.
